The app has been downloaded more than 2,000,000 times, reaches active users in 204 countries, and has users in all 13 Canadian provinces and territories. In August 2026, 247 Labs shipped V4.0: a ground-up rebuild of the product on a modern stack.

The Challenge

An Access Gap Clinics Cannot Close, On a Platform That Had Outgrown Its Foundation

Wait times for initial mental health appointments routinely run months across North America. For someone dealing with mild-to-moderate anxiety today, that gap is the whole problem: they are not sick enough to be prioritized, and not well enough to wait comfortably. Anxiety Canada needed clinically credible support that could work on its own for people who could not reach a therapist, and alongside professional care for people who could.

The second challenge was technical. A product used daily in 204 countries had accumulated years of architecture that made every new capability — additional languages, clinician-facing reporting, organizational deployments, healthcare-grade privacy controls — harder than the last. Modernizing it meant rebuilding the foundation without disrupting the users already relying on it, and without touching the clinical integrity of the content.

The Solution

Ten CBT Tools in One App, Rebuilt From the Ground Up in V4.0

247 Labs designed and built MindShift CBT around techniques with an established evidence base, delivered in plain language that a user can apply without a clinician in the room:

  • Thought Journals — structured entries for identifying, challenging, and reframing anxious thinking (cognitive restructuring)
  • Fear Ladders — graded exposure hierarchies for approaching feared situations one step at a time
  • Chill Zone — guided audio for grounding, mindfulness, paced breathing, progressive muscle relaxation, and sleep
  • Quick Relief Tools — brief coping prompts for moments of acute anxiety or panic
  • Healthy Habits — guidance on sleep, movement, connection, and the lifestyle factors that carry anxiety management
  • Goal-Setting and Mood/Anxiety Tracking — recurring check-ins that make patterns and progress visible
  • Daily Check-In — a consistent self-assessment habit that feeds the tracking view over time
  • Coping Cards — user-created reminders and grounding strategies for stressful moments
  • Community Discussion Board — an actively moderated peer-support space, monitored by the MindShift team
  • Data Export for Clinical Collaboration — structured reports of thought records, fear-ladder progress, and mood data that a user can hand to their therapist or physician

Version 4.0 rebuilt the product on React Native and Laravel and used that foundation to extend what the app can do:

  • A React Native front end and Laravel back end, with the architecture sized to support roughly 10x user growth
  • A newly rebuilt French experience, alongside English (with Spanish, Hindi, and Arabic planned)
  • Regional crisis routing to 9-8-8, the Hope for Wellness Help Line, Kids Help Phone, and provincial services. MindShift is not a crisis service; the routing exists to get users to human help quickly
  • Enhanced clinician data export with clearer progress summaries built for between-session review
  • Organization-specific wellness spaces for schools, employers, and healthcare organizations, with usage and outcome analytics
  • A privacy and security posture aligned to PIPEDA and HIPAA, which is what makes Canadian healthcare deployment and US partnerships possible

The Evidence

A Published Trial, Not Just App Store Ratings

Most consumer mental health apps have no published evidence of effectiveness. MindShift CBT has been studied in a 16-week open-label trial of 380 adults aged 18–80 across Canada and the United States (ClinicalTrials.gov NCT04562948). Participants showed significant improvement across all four primary outcomes: anxiety symptoms, depressive symptoms, functional impairment, and quality of life.

MindShift CBT is not a medical device and is not diagnostic. It is a self-management tool grounded in established CBT practice, designed to sit before, between, alongside, and after clinical care — never instead of it.

Areas of Impact

A Front-Line Digital Support Tool for Anxiety

  • Three populations, one product. Self-managers outside formal care, users waitlisted for therapy, and patients using the app between sessions with their clinician.
  • Reach where care is thinnest. Active users in every Canadian province and territory and in 777 cities and towns, including Northern and remote communities such as Iqaluit, Yellowknife, Inuvik, Whitehorse, Thompson, La Ronge, and Moosonee, where clinical access is limited or unavailable.
  • Built with clinicians in the loop. Data export turns patient-generated records into something a therapist can actually use, so app activity informs treatment rather than running parallel to it.
  • Designed for adolescents, not adapted for them. The app was co-designed with and for users 13 and up, addressing a youth mental health crisis that clinical capacity has not kept pace with.
  • Ready to deploy inside organizations. Branded wellness spaces, outcome analytics, and integration pathways for EHR, insurer, and wellness platforms.
